you can see John’s whole world breaking around him as he see’s the fall happening all over again.

i think part of the reason Sherlock turned away is because thats what he saw to.  he realized moments to late that he hit a trauma point in John.  him being taken away from John and he’s done it all over again.  he can’t bear to look so he turns away

Sherlock does look a little startled there.  ”See, I told you I’d take care of-  Oh, shit.  That’s the devastated face.  In retrospect, this might have been a terrible idea.”
