Auditory Hallucinations: noises or phrases you hear that aren’t really there. They can be so real you’re convinced someone is speaking to you. 

Common Causes:

  • High levels of anxiety
  • Drug use

Sherlock could be under high levels of anxiety. At this point, John isn’t speaking to him. I imagine that caused him quite a lot of strain. He isn’t used to working with anyone besides John. 

OR

Sherlock slipped up while he was away and started using again. Either way, I am not okay.

Honestly I don’t know why everybody’s assuming they’re hallucinations.  I assumed it was just Sherlock’s big old overactive brain chasing after distracting John-related tracks while he was trying to focus something else. 

I do this all the time, including occasionally the snapping at myself when my stupid brain doesn’t want to shut up.  And I KNOW I’m not the only person who does.
